Please find the link to the Royalties Aged Creditors form here. It should be signed, completed and returned to the royalty’s team at this email address: acr@cambridge.org 

  
Before sending the form over to acr@cambridge.org ensure that you have completed the following:  
  
1. The form is completed with your current address & bank details  
2. Your address stated matches the proof of address required & your photo ID is valid and in date  
3. The bank form is signed & dated  
4. The bank details provided are your current/checking account (not a savings or investment account)
